Transitioning from home to long-term care: the experiences of older persons, family members and service provides

PROJECT DESCRIPTION:
The purpose of this study is to interview residents and family members within the first two weeks of living in a long-term care facility to determine those factors that affect (either positively or negatively) the transition from living in the community to living in long-term care. In addition to speaking with residents and family members, the research team is speaking with policy makers and community agencies (CCAC) about the way in which the transition process currently happens, and how it may be improved in the future.

PRACTICAL IMPLICATION
Ultimately, they are aiming to offer policy and practice recommendations that support better transitions for older adults and their family members.
