RESEARCH PROGRAM ASSOCIATION: Optimizing Medications for Seniors
PROJECT DESCRIPTION:
This project examined medication system protocols in the Schlegel Villages. Focus groups and chart audits examined the process of patient identification, medication information on entry to the home, transcribing of medication orders, drug standardization, storage and distribution, staff education, and patient safety. Recommendations for improvement were identified, including the need for better information upon entry to improve medication reconcilliation, making medication passes more free of interruption, more education, and more standardization in pain assessment.
