Building Community Capacity in Interprofessional Care to Improve Health Care for Seniors

"Interprofessional Care in Action: From Theory to Practice"
The "Interprofessional Leadership Project" is one of 41 projects funded by HealthForceOntario in 2009 and is a collaborative effort between University of Ontario Institute of Technology (UOIT) & the Health Education Technology Research Unit (HETRU), The Village of Taunton Mills in Whitby and the Schlegel-UW Research Institute for Aging (RIA).
Key project elements include interprofessional education & supported practicums, overview by a project advisory group, and the exploration of navigation for seniors.
HETRU at UOIT adapted their evidence-based IP framework, "The Compass Model" which included the development of a Core Competency Framework on interprofessional education and complex health video simulations showcasing interprofessional collaboration to deliver Education Workshops for 30 'Leadership Participants' from the community who were chosen to represent a diversity of health provider backgrounds and agencies that currently deliver services for seniors.
Each of the health providers completed 18 hours of 'in-class' education and developed a practicum project (with support) where they used their new knowledge to implement a new IP practice. Group One practicum projects ran from July to December 2009 and outcomes were shared via poster presentations which are available here.
